- NYC registered programs will order naloxone and Fentanyl Test Strips (FTS), by completing the New York City Overdose Prevention Supplies Order Form

Required of NYC-registered programs: Quarterly report & community usage reports (submit online, log-in above). In addition, monthly reporting to NYC Department of Health and Mental Hygiene.
